Highmark Health is seeking a highly motivated Transplant Care Nurse RN with experience in Stem Cell and Bone Marrow transplantation to join our team. In this role, you will utilize your expertise in case management and utilization management to provide exceptional care and support to patients and their families. Provide compassionate and skilled nursing care to patients undergoing transplant procedures remotely. Utilize expertise in case management and utilization management to ensure the highest quality of care for patients. Communicate effectively with patients and their families to provide necessary support and education throughout the transplant process. Monitor and assess patient's physical and emotional well-being during and after transplant procedures. Collaborate with interdisciplinary team to develop and implement individualized care plans for patients. Maintain accurate and detailed patient records, including documenting patient progress and any changes in treatment plans. Adhere to all medical protocols and standards of care to ensure patient safety and positive outcomes. Utilize critical thinking skills to identify and address potential complications or concerns during the transplant process. Continuously evaluate and improve processes to ensure the highest level of patient care and satisfaction. Stay up-to-date on advancements and best practices in transplant nursing, particularly in the areas of Stem Cell and Bone Marrow transplantation. Serve as a resource and advocate for patients and their families, providing support and guidance throughout their transplant journey. Demonstrate a strong commitment to ethical and professional standards in all interactions with patients, colleagues, and healthcare providers. Maintain confidentiality and adhere to all HIPAA regulations and guidelines. Participate in ongoing training and education to enhance knowledge and skills related to transplant nursing.
