We're building a new model: one that pairs skilled Health Advocates with clinical expertise and technology to deliver coordinated, whole-person care at scale. Physicians are essential to that model.
The Role
As a Remote Telehealth Physician at Aviator, you'll work alongside Health Advocates and nursing staff to ensure patients receive high-quality, coordinated care - wherever they are. You'll bring your clinical expertise to help patients and families navigate chronic illness, make sense of complex care decisions, and achieve better health outcomes.
This is also a ground-floor opportunity to shape how Aviator's care model evolves. You'll contribute directly to building systems and approaches that can scale nationally.
What You'll Do
• Provide clinical oversight and expertise for patients managing chronic conditions
• Partner with Health Advocates to develop and monitor holistic, individualized care plans
• Identify and prioritize patient needs to ensure continuity and quality of care
• Incorporate social determinants of health (SDOH), including food security, transportation, and housing, into care planning
• Leverage AI-enabled tools and technology platforms to deliver care efficiently
What We're Looking For
• Active licensure in an IMLC member state, with willingness to obtain additional state licenses as needed
• Board certification in Internal Medicine, Family Medicine, or a closely related specialty
• Experience caring for patients with chronic conditions, ideally in a primary or longitudinal care setting
• Strong clinical judgment with the ability to coordinate across multiple care team members
• Excellent communication skills — you listen carefully, explain clearly, and guide with empathy
• Comfort with technology and the ability to learn new systems quickly
• A proactive, solutions-oriented mindset; you're energized by building, not just maintaining
Strong preference for candidates currently licensed in 30+ states.

Compensation: Competitive hourly rate, commensurate with experience
