Northwest Kidney Centers seeks a Chronic Kidney Disease (CKD) Educator to join the Chronic Kidney Disease Program in a per-diem, hybrid position.

Purpose:

The Chronic Kidney Disease (CKD) Educator is responsible for providing education, support, and assistance to facilitate informed decision-making, access to care and successful self-management for pre-dialysis patients; with the goal of improving outcomes related to modality choice (including palliative care), access placement, and transplant. The position is responsible for teaching CKD classes, assisting in development and implementation of educational programs for patients, NKC staff, and the community. Internally, the CKD Educator serves as a liaison, consultant, and resource for the internal Interdisciplinary Team Members regarding dialysis patient education. Externally, this position will perform outreach/education to area Nephrologists, healthcare providers, individuals impacted by CKD, and the lay public.

Shift: Per-Diem/Variable, Mon-Fri.

Duties and Responsibilities: The ideal candidate will:

The CKD Educator must a registered nurse or licensed social worker. 2 years of previous clinical patient education experience in the renal field is required. The CKD Educator must demonstrate competency in clinical practice, adult education, interpersonal communication skills, outreach/networking with stakeholders and leadership.
