Job Overview: Remote Overnight Full-Time NY Licensed Nurse Practitioner
We're seeking an experienced and skilled Nurse Practitioner to join our team on a full-time overnight remote basis. As a Nurse Practitioner at Vesta Healthcare, you'll play a critical role in triaging inbound calls, conducting care coordination, and applying your clinical expertise to manage the care of at-risk adults. Your work will be pivotal in ensuring that our members receive high-quality, patient-centered care.
Key Responsibilities
- Work overnight shifts (11pm ET - 8am ET) Monday to Friday, with your last shift ending at 8am on Saturday morning.
- Triage inbound calls from members, assessing actions to be taken and ensuring timely, appropriate care.
- Conduct care coordination, recommending and identifying cost-effective, research-based treatments and interventions.
- Utilize strong clinical skills in physical assessment and chronic disease management for at-risk adults.
- Develop and update member-specific care management plans, ensuring individualized care.
- Serve as a consulting resource on care management practices as needed.
- Participate in meetings and training sessions to stay updated on best practices and company initiatives.
- Provide education to members, caregivers, and facilities on care management and health-related topics.
- Monitor the quality of member care, updating plans of care as necessary.
Essential Qualifications
- Certified and licensed as a Nurse Practitioner in good standing in the state of New York.
- Master's or doctoral degree from an accredited institution for nurse practitioners.
- Medicare participation and the ability to have the company bill for services on your behalf.
- Certification from ANCC (or equivalent) as an Adult, Family, Geriatric, and/or Acute Nurse Practitioner.
- 1+ years of Nurse Practitioner experience, with a preference for those qualified for independent practice in their licensed jurisdiction.
- 1+ years of telephonic triage or equivalent experience.
- 2+ years of clinical experience working with complex adult populations.
- Ability to practice independently with minimal clinical support.
- Comfort using technology such as Google Suite, multiple EMRs, and Slack.
- A private workspace with a computer and reliable internet connection for remote work.
Preferred Qualifications
- Experience working in home care and/or family medicine, geriatrics.
- Background working within a clinical team environment.
- Strong organizational skills, including the ability to prioritize tasks effectively.

Our competitive salary range is $130K - $140K annually, based on factors such as geographic location, work experience, market conditions, education/training, and skill level.
